Physical Therapist Home Health

Medi Home Health & Hospice , a division of Medical Services of America, Inc., currently seeks an experienced Full-time Physical Therapist for our Home Health patients in Pulaski, VA. This position will cover Pulaski and Wytheville Counties.
Provide physical therapy services to patients under appropriate guidelines.
Implement and conduct treatment as outlined in treatment plan.
Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments and procedures within the plan of care.
Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.
Interpret and communicate evaluation findings to patients, family and other staff members.
Qualifications and Skills
Valid Physical Therapy license in the state of VA.
Valid Driver's License.
Current CPR certification.
Minimum of one year verifiable physical therapy experience.
Home Health experience preferred.
